Applications for Financing $75,000 and above
(For Canada Financing Only)

Applications for more than $75,000 for Canada should include the following:

1. Completed Customer Application Including
 
Complete Name & Address
Owner/Officer Names & Social Insurance Numbers
Bank Name, Checking Account Number, Contact Person (if available) and Phone Number
Date Applicant’s Ownership of Business began
Landlord or Mortgage Holder Information
Signature of Each Owner/Officer
2. Copies of Three Most Recent Months Bank Statements for Applicant(s)
3. Copy of Equipment Proposal Listing:
 
Make & Model Numbers of the Equipment
New or Used Designation of Each Piece of Equipment
Cost Per Piece of Equipment
Freight & Installation Must be Separate
Proposal Must be Signed by Applicant(s)

APPLICATION MORE THAN $50,000 OR OWNERSHIP LESS THAN 2 YEARS – INCLUDE ITEMS 1 THROUGH 3 PLUS THE FOLLOWING:

1. Most Recent Two Years Complete Federal Tax Returns on all Principals (Owners, Partners, Shareholders, or Members Owning More than 20% of Applicant)
2. Most Recent Two Years Complete Federal Tax Returns on Applicant if Different than Item 1
3. Personal Financial Statement of Applicant and, if Different than Applicant, all Principals
4. Copy of Premises Lease
5. Completed Underwriting Questionnaire
6. Proforma/Projected Financial Information of Business
